Health

Our Community Health Workers are trusted members of our community who bridge the gap between individuals and the systems designed to serve them.

Healthcare systems, benefits programs, and social services were built to help people — but navigating them can feel like a full-time job. ELLA’s Community Health team removes those barriers by providing the guidance, advocacy, and coordination that turns a confusing system into actual care.

Community Health Worker (CHW) Services

Our CHWs are trusted, culturally connected members of our community who provide outreach, education, and direct support. They reduce barriers to care by meeting clients where they are — physically and emotionally.

CHW services include home visits, community outreach events, health education, and long-term relationship-based support.

Benefits Navigation

Navigating public benefits is complex and often overwhelming. Our team helps individuals and families apply for and maintain the benefits they're entitled to.

  • Medi-Cal — eligibility, enrollment, and renewal
  • Medicare — Part A, B, C, and D navigation
  • ACA Marketplace — plan selection and enrollment
  • TriWest/TRICARE — military family coverage
  • CalFresh, CalWORKs, and other safety net programs

Referrals to Specialists & Community Resources

You shouldn't have to figure out who to call alone. ELLA maintains a current network of trusted specialists, clinics, legal aid organizations, housing programs, and community resources across all four service counties.

We follow up to ensure warm handoffs — not just a phone number.

Care Coordination

For clients navigating multiple systems or services simultaneously, our care coordinators help create a coherent plan — connecting medical, behavioral health, housing, and social service providers around a shared goal.

Care coordination is especially valuable for clients with complex needs, chronic conditions, or multiple open cases.

Service Area

ELLA’s Community Health program serves clients across three Southern California counties. Services are available in-person, via telehealth, and through community-based outreach events.
